1. Data controller

The controller of your personal data is PPDENT Sp. z o.o., with its registered office at ul. Tadeusza Zawadzkiego 148/2, 71-246 Szczecin, Poland, entered in the register of entrepreneurs of the National Court Register under KRS no. 0000877799, NIP (tax ID) 8522669462, REGON 387898350 (the “Controller”).

You can contact the Controller in all matters concerning personal data by phone on +48 518 225 305 or +48 91 439 40 01, or by e-mail at gabinet.poludzien@gmail.com. The Controller has not appointed a data protection officer.

2. What data we process and why

When you use this website, we process the technical data necessary for its operation and security: your IP address, browser and device data, and the server logs kept by our hosting provider. The legal basis for this processing is our legitimate interest pursuant to Art. 6(1)(f) GDPR.

We measure how the site is used with Google Analytics 4 only with your consent (Art. 6(1)(a) GDPR). Until you consent in the cookie banner, the Google Analytics script does not load at all and no data is sent to Google.

We also use Cloudflare Web Analytics, a privacy-focused analytics service from our hosting provider Cloudflare, to measure aggregate site traffic: page views, referring sites, and the general browser, device and country information derived from your request. The service does not use cookies, does not store your IP address, does not track you across websites and does not build individual profiles. Because it uses no cookies and does not identify you, it operates on the basis of our legitimate interest (Art. 6(1)(f) GDPR) and does not require your consent.

If you contact us by phone or e-mail, we process the data you provide (such as your name, phone number, e-mail address and the content of your message) to handle your enquiry. The legal basis is Art. 6(1)(b) GDPR, that is, steps taken at the request of the data subject prior to entering into a contract, such as arranging an appointment, or our legitimate interest (Art. 6(1)(f) GDPR).

The appointment booking widget (ZnanyLekarz) loads only after you click it. When the widget loads, ZnanyLekarz receives your IP address and technical browser data. Data entered in the widget is transmitted directly to the ZnanyLekarz.pl service, whose operator, DocPlanner, is a separate and independent controller of that data. Its processing is governed by the ZnanyLekarz privacy policy.

The contact page embeds a Google Maps map. The map loads only after you click it. When the map loads, Google receives your IP address and technical browser data. This processing is based on our legitimate interest (Art. 6(1)(f) GDPR) in helping patients reach the practice.

3. Legal bases

Consent (Art. 6(1)(a) GDPR): Google Analytics. You may withdraw your consent at any time. Legitimate interest (Art. 6(1)(f) GDPR): operating and securing the site, aggregate Cloudflare analytics, the map, and handling correspondence. Steps taken prior to entering into a contract (Art. 6(1)(b) GDPR): when you contact us to arrange an appointment.

4. Cookies and similar technologies

The site itself sets no cookies of its own. Your cookie-banner decision is stored in your browser’s local storage (localStorage). The record contains the consent version and date and expires after 12 months, after which we will ask you again. Our hosting provider (Cloudflare) may set short-lived cookies strictly necessary for security purposes, such as bot protection.

Google Analytics cookies (_ga and related) are set only after you consent. You can change or withdraw your consent at any time via the “Cookie settings” link in the page footer. After withdrawal, analytics no longer runs on subsequent page views, and you can delete previously stored cookies in your browser settings. Withdrawal of consent does not affect the lawfulness of processing carried out before the withdrawal.

5. Recipients

Google Ireland Ltd. / Google LLC: Google Analytics (only after consent) and the Google Maps embed on the contact page.

Cloudflare, Inc.: website hosting and security (CDN), and cookieless aggregate traffic measurement (Cloudflare Web Analytics).

DocPlanner (ZnanyLekarz.pl): appointment booking, an independent controller of the data entered in the widget.

6. Transfers outside the EEA

Google LLC and Cloudflare, Inc. are established in the United States and are certified under the EU-US Data Privacy Framework, which is covered by a European Commission adequacy decision. Standard contractual clauses apply in addition. Apart from the cases described above, we do not transfer personal data to third countries.

7. Retention

Google Analytics data is retained for up to 14 months. Your cookie decision is stored for 12 months in your browser, or until you delete it earlier. Correspondence is kept for as long as necessary to handle your enquiry and, where justified, until any potential claims become time-barred under the applicable limitation periods. Technical server logs at our providers are retained for short periods in accordance with their policies.

8. Your rights

You have the right of access to your data (Art. 15 GDPR), the right to rectification (Art. 16), erasure (Art. 17), restriction of processing (Art. 18) and data portability (Art. 20), the right to object (Art. 21), and the right to withdraw consent at any time, without affecting the lawfulness of processing carried out before the withdrawal.

Right to object: where processing is based on our legitimate interest (Art. 6(1)(f) GDPR), you may object at any time on grounds relating to your particular situation (Art. 21(1) GDPR).

You also have the right to lodge a complaint with the President of the Personal Data Protection Office (UODO), ul. Stawki 2, 00-193 Warsaw, Poland. You may also complain to the supervisory authority of your habitual residence or place of work.

9. Voluntary provision and automated decisions

Providing your data is voluntary. You can use the site in full without giving analytics consent. We make no decisions based solely on automated processing, including profiling, that would produce legal effects concerning you or similarly significantly affect you.

10. Changes

This policy may be updated, for example when the tools we use change. The current version is always available on this page.
